Stress fractures are small breaks in the bone that are caused by repetitive stress. They typically occur due to overuse, forcing the bones of the foot or ankle to continually absorb the full impact of each step taken. If the intensity of the activity increases before the stress fracture has properly healed, it can cause a full fracture.

Treatment can vary depending on the individual and the degree of injury. The primary way to treat a stress fracture is to rest the hurt foot. Some fractures will heal quickly with only a little bit of rest, while others may require a long rest period and the use of crutches, immobilization, or physical therapy. Under certain circumstances, surgery may be required to install support pins around the fracture to assist in healing.

If you are undergoing a new exercise regimen in running or some other kind of high impact activity, set incremental goals on a weekly basis so you can build up muscle strength. Make sure to wear supportive shoes to better protect you feet.

Faulty Pronation Can Cause Foot Problems

Pronation, a natural part of walking and running, specifically involves how you place the heel on the ground and the subsequent transfer of weight to the rest of your foot. People with normal pronation have a normal arch and the weight is transferred in line. People with flat feet generally overpronate, causing the arch to roll inward. With underpronation, the foot rolls outward as the weight is distributed. Many foot problems are associated with over- or under-pronation. Among them are bunions, arch pain, plantar fasciitis, Achilles tendonitis, calluses and corns, ankle sprains and hammertoe. You may have pronation problems if your heels or knees turn inward while standing, if you develop bunions or flat feet, or if you wear out the soles and heels of your shoes quickly. Obesity, pregnancy or repetitive pounding of the feet on a hard surface contribute to the problem, along with wearing high heels and standing for long periods in them. One solution for pronation problems is wearing custom orthotics to help distribute the weight properly. A visit to your podiatrist is suggested to have a full diagnosis of your gait and consequent foot problems, and to be fitted for the appropriate shoe inserts.

Biomechanics in Podiatry

What is a Podiatrist?

A podiatrist is a Doctor of Podiatric Medicine who treats the foot, ankle, and related structures of the leg. If you are having any pain, injuries, or abnormalities in these areas, it is best that you seek help from a podiatrist.

Podiatrists complete four years of training in a podiatric medical school. Their training is like that of other physicians, and they may go on to complete a fellowship training after a residency training. Some podiatrists are board certified meaning they have advanced training, clinical experience, and have taken an exam to prove their skills. Certifying boards for podiatry are the American Board of Foot and Ankle Surgery and the American Board of Podiatric Medicine. Heel Pain

Have you ever gotten up from a chair or out of bed in the morning, and upon taking that first step, feel like your heel has stepped on a tack? Many people experience a feeling of sharp pain which radiates into their arch from their heel and which does not allow them to put their heel on the floor. Sometimes they need to sit back down, stand only on their toes and use the wall for balance. If you can take a few steps, it seems to go away and lessen, allowing you to then resume your activity. Later, throughout your day and after a period of rest, it can happen again. If this sounds familiar you may be suffering from your first attack of heel pain.

Heel pain is a debilitating condition that affects day to day activities. Running and walking both causes stress on the heel because the heel is the part of the foot that hits the ground first. This means that the heel is taking on your entire weight. Diagnosis and treatments for heel pain can be easily found through your podiatrist.

Plantar Fasciitis

One of the main causes of heel pain is a condition known as plantar fasciitis. The plantar fascia is a band of tissue that extends along the bottom of the foot, from the toe to the bottom of the heel. A rip or tear in this ligament can cause inflammation of these tissues, resulting in heel pain. People who do not wear proper fitting shoes are often at risk of developing problems such as plantar fasciitis. Unnecessary stress from ill-fitting shoes, weight change, excessive running, and wearing non-supportive shoes on hard surfaces are all causes of plantar fasciitis.

Achilles Tendonitis

Achilles tendonitis is another cause of heel pain. Similar to plantar fasciitis, inflammation of the Achilles tendon will cause heel pain due to stress fractures and muscle tearing. A lack of flexibility of the ankle and heel is an indicator of Achilles tendonitis. If left untreated, this condition can lead to plantar fasciitis and cause even more pain on your heel.

Heel Spur

A third cause of heel pain is a heel spur. A heel spur occurs when the tissues of the plantar fascia undergo a great deal of stress, leading to a separation of the ligament from the heel bone entirely. This results in a pointed fragment of bone on the ball of the foot, known as a heel spur.

Cuboid Syndrome Causes

Cuboid syndrome is a condition that is caused by a partial dislocation of the cuboid bone, located in the center of the foot. When this bone is knocked out of place during an injury, it can cause symptoms such as pain along the outside of the foot, swelling, sensitivity, difficulty walking, and a reduced range of motion. Cuboid syndrome is most frequently caused by repetitive overuse during athletic activities, like tennis, basketball, or dancing. When excessive stress is placed on the feet over and over, cuboid syndrome is more likely to occur. This condition is often associated with sprained ankles. Having altered foot biomechanics, a pronated gait,or a tendency to wear ill-fitting shoes can also increase your risk of injuring the cuboid bone. Cuboid syndrome is a common cause of lateral foot pain, which is pain on the outside of the foot. The condition may happen suddenly due to an ankle sprain, or it may develop slowly overtime from repetitive tension through the bone and surrounding structures.

Causes

The most common causes of cuboid syndrome include:

  • Injury – The most common cause of this ailment is an ankle sprain.
  • Repetitive Strain – Tension placed through the peroneus longus muscle from repetitive activities such as jumping and running may cause excessive traction on the bone causing it to sublux.
  • Altered Foot Biomechanics – Most people suffering from cuboid subluxation have flat feet.

Symptoms

A common symptom of cuboid syndrome is pain along the outside of the foot which can be felt in the ankle and toes. This pain may create walking difficulties and may cause those with the condition to walk with a limp.

Diagnosis

Diagnosis of cuboid syndrome is often difficult, and it is often misdiagnosed. X-rays, MRIs and CT scans often fail to properly show the cuboid subluxation. Although there isn’t a specific test used to diagnose cuboid syndrome, your podiatrist will usually check if pain is felt while pressing firmly on the cuboid bone of your foot.

Treatment

Just as the range of causes varies widely, so do treatments. Some more common treatments are ice therapy, rest, exercise, taping, and orthotics.

All About Cuboid Syndrome

Though cuboid syndrome predominately affects athletes, non-athletes can suffer from it too. Cuboid syndrome is also called cuboid subluxation or cuboid fault syndrome, and occurs when a joint or ligament near the cuboid bone of the foot becomes damaged, or when the bone itself is dislodged from its natural position. Pain may be persistent, or come and go, and it is usually marked by the outside of the foot. Cuboid syndrome, unless severe, can be difficult to diagnose. A doctor will likely ask questions about how long the pain has been present, and will apply pressure on the cuboid bone to determine the origin of pain.

There are a number of causes that can lead to the syndrome. Due to athletic activities, repeated stress placed on the foot can cause cuboid subluxation. Ballet dancers, runners, and other athletes often develop this condition. Basketball or tennis players may also develop this condition, as they place stress on their feet while moving side to side. Cuboid syndrome can often develop over time; however it can come out of a sudden injury as well. Over pronation, or other problems with feet, can exacerbate the condition if not corrected.

Among podiatrists, there is some disagreement about the treatment, as well as the definition of cuboid syndrome. Some see the injury as an injury to the ligaments located nearby the cuboid bone, while others believe it refers to the dislocation of the calcaneal-cuboid joint only. Treatment opinions differ as well. Although it can be treated by manipulation in order to reposition the bone, this must be done with extreme care in order to avoid injury. Some doctors, however, prefer treatment through the use of orthotic pads, designed to keep the bone in its place. Effectiveness of these treatments may vary, according to the severity of the injury.

When you experience side foot pain, it is important that you seek medical assistance. If a subluxed cuboid is caught and treated early, treatment is usually successful, and individuals may begin activities such as sports when the pain subsides. If left untreated, the pain will worsen, and the condition could cause permanent damage.

It is believed that many foot problems are caused by wearing the wrong kind of shoe. Here are some things you can do to help keep your feet healthy by choosing the right kind of shoe for your individual foot. Start by outlining your feet on a piece of paper and bringing that with you when you are buying shoes. Go in the afternoon or evening, after your foot has had a chance to expand throughout the day. When you get to the store, have your foot measured or do it yourself. Be sure to stand up to make sure each foot is measured accurately. If your feet vary in size, choose the larger size. Look for shoes that have good arch support, no rough inner seams, at least 1/2” of space between your longest toe and the tip of the shoe, and a structured back to hug your heel. When you find a shoe you are interested in, check to see if it matches your foot shape before you try it on. Place the shoe on top of the outline you brought: if it covers the outline completely, that means it might be a good match. Try the shoe on and walk around to make sure nothing pinches or rubs your foot. For additional tips on finding shoes that are right for your feet, consult with a podiatrist.

Proper Shoe Fitting

Shoes have many different functions. They cushion our body weight, protect our feet, and allow us to safely play sports. You should always make sure that the shoes you wear fit you properly in order to avoid injuries and deformities such as: bunions, corns, calluses, hammertoes, plantar fasciitis, stress fractures, and more. It is important to note that although a certain pair of shoes might be a great fit for someone else, that doesn’t mean they will be a great fit for you. This is why you should always try on shoes before buying them to make sure they are worth the investment. Typically, shoes need to be replaced ever six months to one year of regular use.

Tips for Proper Shoe Fitting

  • Select a shoe that is shaped like your foot
  • Don’t buy shoes that fit too tight, expecting them to stretch to fit
  • Make sure there is enough space (3/8” to ½”) for your longest toe at the end of each shoe when you are standing up
  • Walk in the shoes to make sure they fit and feel right
  • Don’t select shoes by the size marked inside the shoe, but by how the shoe fits your foot

The shoes you buy should always feel as good as they look. Shoes that fit properly will last longer, feel better, and improve your way of life each day.
